Application Procedure
Grants will be approved on
a rolling basis. Prior to a not-for-profit organization submitting a
full proposal, applicants must complete the pre-application form.

Upon receipt of the pre-application,
Foundation staff will inform applicants whether a full application
should be submitted or whether the project will not be considered,
usually within a timeframe of four to six weeks.
Mountainside Health Foundation uses the
New York/New Jersey Area Common Application Form, which may be downloaded
by clicking the button below.

Submit one copy of your
application electronically to
ksmith@mshfoundation.org and mail a hard copy to the
Foundation offices with all required materials
attached. (The Common Application lists the required
attachments.) |
Charitable Funding
Guidelines
The Foundation is an independent,
self-governing public charity based in Montclair, NJ. We seek to create
healthier communities by supporting and advancing good health, wellness
education, and disease prevention and treatment. Among the Foundation’s
top priorities is developing provider partnerships to more effectively
respond to the health needs of the local community.
The Foundation will consider proposals from charitable organizations
determined by the Internal Revenue Service to be tax exempt under
Section 501 (c)(3) of the code, which are located in, or providing
services to, one or more of the following communities: Montclair, Glen
Ridge, Essex Fells, Verona, Cedar Grove, Clifton, Bloomfield, Little
Falls, Nutley, Roseland, Fairfield, Caldwell, West Caldwell, North
Caldwell, and West Orange.

We currently have a
Request for Proposals within Mental

Grant
Cycle
Applicants can expect to receive a final determination on their requests
within three months of submission. In the interim, in order for us to
fully understand the goals of your proposal, Foundation staff may
contact you to:
· Request additional materials
· Schedule a telephone call to discuss the proposal
· Request a meeting at the Foundation offices
· Schedule a site visit at your location
After the Foundation staff has informed you that your pre-application
has been approved, you will be asked to submit a full proposal for
consideration by the Board of Trustees. A full proposal must be received
by the Foundation 4-5 weeks prior to a scheduled Board meeting. Thus:
· Proposals to be considered in February are due the second Friday in
January
· Proposals to be considered in May are due the first Friday in April
· Proposals to be considered in July are due the second Friday in
June
· Proposals to be considered in October are due the first Friday in
September
· Proposals to be considered in December are due the first Friday in
November
